I, the undersigned, as the parent/guardian of the above-named participant, give my full consent for my child to participate in the Teen Beauty Bootcamp organized by Glambox University. I understand that the activities will include workshops on hair braiding, makeup, skincare, and nail basics.*

In the event of an emergency, I hereby authorize the staff of Glambox University to secure any necessary treatment for my child. I also agree to be responsible for any costs associated with such treatment.*

I grant permission for Glambox University to take photographs and videos of my child during the Bootcamp and to use these images for promotional purposes on their website, social media, and other marketing materials.*
